Overview
The Clinical Pharmacist will be responsible for reviewing patient charts to assess medication therapy, ensuring the delivery of safe and effective care tailored to patient needs, including age-specific requirements. As a Clinical Pharmacist, you will work closely with healthcare teams to optimize medication management and patient outcomes. You will oversee sterile, non-sterile, and hazardous compounding, ensuring compliance with USP standards and other state and federal requirements. The Clinical Pharmacist will collaborate closely with Advanced PharmacyΒ Technicians and Clinical Pharmacy Specialties to Β provide comprehensive care
What you will do
Review, validate, and assess patient-specific medication orders, monitoring patient profiles for accurate dosing and therapy.
Identify, interpret, and escalate potential medication concerns to providers or pharmacy leadership as appropriate.
Supervise and perform sterile, non-sterile and hazardous compounding, packaging, and dispensing of pharmaceuticals in accordance with relevant laws and regulations.
Lead pharmacy operations, including order review, controlled substance management, pharmacy automation, and compliance with state and federal regulations.
Establish and maintain a safe medication environment, ensuring the implementation of best practices for patient care.
Actively participate in core clinical pharmacy programs, including IV to PO conversion, renal/hepatic dosing, therapeutic interchange, antimicrobial stewardship, anticoagulation management, and more.
Monitor and manage medication therapy, including identifying drug-drug interactions, duplicate therapy, and non-formulary medication needs.
Collaborate with multidisciplinary teams to support evidence-based patient care and enhance clinical outcomes.
Qualifications
Education
Doctoral Degree, Pharmacy (Required) or
Bachelors Degree, Pharmacy (Required)
Experience
1 year Prior pharmacy experience in area of practice related to the position (Preferred)
1 year PGY1 Residency (Preferred)
Licenses and Certifications
Licensed Pharmacist - Virginia Department of Health Professions (VDHP) within 30 Days(Required)
CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Preferred)
Board Certified Pharmacy Specialties (BCPS) - Board of Pharmacy Specialties (Preferred)
