Pharmacist - Clinical Services
Summary
Primary responsibilities are to work in partnership with field teams and shared services, serving as the Clinical Pharmacist. This position will also provide support to the field and on-site Pharmacy Teams by providing centralized clinical services as appropriate. Within this scope of practice, the pharmacist will also need registration in multiple states as appropriate for support. Additionally, the pharmacist will be responsible for oversight of technician functions within Shared Services as appropriate by state regulation.
Essential Functions
• Enrolls patients into the specialty pharmacy program, working closely with them to support them for the life of their therapy to help them achieve the best clinical outcome throughout their therapeutic journey.
• Reviews records of patients to determine the appropriateness of medication therapy.
• Develops effective medication plans that minimize the risk of adverse side-effects.
• Advises patients on the correct administration of medications.
• Contacts Physicians for clarification or questions on any orders or prescriptions written by the Physician.
• Provides Clinical advice to Physicians regarding medication treatments.
• Obtains any and all information necessary from Physicians to ensure accurate and safe dispensing of medications to the patient.
• Provides patient-specific consultation on all aspects of their medication regimen.
• Maintains appropriate records and computer entry of prescriptions and patient information.
• Documents patient interventions with adequate detail in both clinical and non-clinical records as appropriate.
• Reports adverse events to the appropriate collector, such as MedWatch or a manufacturer, in a timely manner.
• Coordinates clinical projects, assisting with formulary decisions when necessary, and ensures billing compliance.
• Assists in working to prevent and/or resolve drug-related issues, and provides clinical guidance and drug information to patients, physicians, nurses, and other healthcare professionals.
• Develops and maintains an escalation protocol on situational specific matters that encompasses the understanding of when it is appropriate to involve the Pharmacist-In-Charge and/or the Regional Director or Associate Director of Pharmacy Operations.
• Documents, tracks, and follows up on critical action items on a daily basis.
• Works closely with the Quality Team to ensure maintenance and completion of clinical and operational audits.
• Ensures consistency and accuracy of clinical and operational documentation within the pharmacies dispensing and patient management systems.
• Addresses escalated complaints and resolves issues.
• Performs duties in accordance with established SOPs and aligned with the core values of the company in the best interest of our clients.
• Enforces all federal, state, and local laws and regulations pertaining to drug dispensing and patient privacy, as well as company policies and procedures.
• Performs other duties as assigned.
What You’ll Bring
Basic Qualifications:
• Pharm.D. or degree from an accredited college of pharmacy.
• Successful completion of Texas state pharmacy board examination.
• Pharmacist license current and in good standing in the state of Texas.
• Up-to-date on continuing education requirements.
Preferred Qualifications:
• One year of experience working with patient management program preferred
• Knowledge of the specialty pharmacy industry preferred
• PGY-1 or 2 residency program preferred
Competencies
• Demonstrates proficiencies in specialty disease states, such as Transplant, Hepatitis C, HIV, Autoimmune, Oncology, Pain Management, etc.
• Comprehends federal, state, and local laws and regulations applicable to the practice of pharmacy.
• Is familiar with workflow concepts relating to the practice of retail pharmacy.
• Shows full understanding of controlled substance rules and regulations as dictated by the Drug Enforcement Agency, as well as state and local laws and regulations.
• Detailed understanding of REMS programs relevant to specialty pharmaceutical products.
• Understands the limited distribution process for specialty pharmaceutical products.
• Has sufficient knowledge, training, and expertise in pharmacotherapeutics to oversee the pharmacy patient management program.

Work Environment and Physical Demands:
• This position operates in a health care setting and requires frequent standing and walking to storage shelves and filling counters to fill prescriptions.
• The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position. While performing the duties of this job, the employee is regularly required to talk and hear. The ability to see distinctions in colors and shapes is also required. This position is moderately active and requires standing and walking for the duration of the shift, and lifting up to 25 lbs.
