Position: Clinical Pharmacist - PA (1099 Contractor)
The Clinical Pharmacist implements Monogram’s medication therapy management (MTM) program, ensuring appropriate selection and use of medications by our patients. As a 1099 contractor, the Clinical Pharmacist will perform timely written comprehensive medication reviews (CMRs) by utilizing patient-provided medication lists, prescription claims, and available clinical information (lab values, etc.). The pharmacist will create drug therapy action plans based on findings and medication recommendations when appropriate for Monogram Health patients, then communicate these recommendations to providers and clinical staff at Monogram Health and in the community. The pharmacist will conduct telephonic consultations with medical providers when medication changes appear necessary and speak with patients about medication use concerns. The pharmacist will work collaboratively with their local Market Physician Executive (POD physician) and clinical staff (e.g., Advance Practice Providers (APPs), nurses, and social workers) to integrate medication management into Monogram Health’s overall care model and to ensure that field clinicians are deploying practices to optimize patient medication use and outcomes.
