Provides primary medical care, including preventative services, as well as diagnosis and treatment of acute and chronic illness within licensed scope of practice and granted privileges for both pediatric and adult patients.
Includes physical examinations, ordering and interpretation of diagnostic tests and imaging, performing indicated medical procedures, prescribing medications, referral to specialty physicians and/or health resources as indicated
Ensures the delivery of all medical care is consistent with regulatory, accreditation, and professional standards, including clinical policies and quality initiatives
Participates in the review and development of clinical policies, protocols and standing orders
Actively engages in compliance and quality improvement activities, including but not limited to, peer review, patient-centered medical home, UDS reporting, PDSA cycles, SOAP notes, other value-based care projects
Attends all scheduled provider and all-staff meetings and trainings, unless excused by CMO
Ensures accuracy, timeliness, and appropriate confidentiality with patient electronic documentation per contract
Conducts all scheduled clinic shifts in person at assigned clinic location (Northview Heights and/or Middle Street offices) and completes virtual visits as needed.
Develops and ensure ongoing, positive relationships and collaboration with all health center staff
Meets professional behavioral expectations for attendance, appearance, professionalism, communication
Trains, mentors and supervises students, trainees, and mid-levels as assigned
Works with a multidisciplinary team
Identifies barriers to self-management plan and meeting goals
Consistently practices universal precautions
Potential exposure to biohazardous materials
Prolonged sitting and/or standing and moving about throughout the workday
Willingness to participate in a minimum of 2 hours of community outreach per year
Is punctual and ready to participate in clinic care at start of each clinic session
Participates in morning patient care huddles, up to 10 minutes outside of patient care hours
Comfort working with the NSCHC faith-based mission to provide care for all who request it, without judgement
Strong computer skills, including Word, Excel, and PowerPoint
Requisite skills and ability to perform medical care
Demonstrate a high level of problem solving skill to better serve patients and staff
Ability to utilize computers for data entry and information retrieval
QUALIFICATIONS (Required for employment)
M.D. or D.O. from an accredited educational institution
Licensed to practice medicine in the State of Pennsylvania
Maintains requisite CME credits for area of specialty
Board-certified/Board-eligible
Active DEA license required
Current provider card in Basic Life Support (BLS)
Clearances: Act 33, 34, 73
Annual COVID vaccination encouraged
Annual influenza vaccine required
Must obtain or provide proof of (within 14 days of hire): PPD, Tdap, Hepatitis B immunity, MMR immunity, varicella immunity
