This role is ideal for a provider who is passionate about delivering high -quality, relationship -based care in a Direct Primary Care (DPC)<\/b> environment while serving a diverse community.

Responsibilities

• Provide comprehensive primary care services to patients across the lifespan
  <\/p><\/li>

• Deliver compassionate, culturally responsive care in a patient -centered setting
  <\/p><\/li>

• Conduct assessments, diagnose conditions, develop treatment plans, and prescribe medications as appropriate
  <\/p><\/li>

• Educate patients and families on preventive care, wellness, and chronic disease management
  <\/p><\/li>

• Document care accurately and efficiently using established clinical workflows
  <\/p><\/li>

• Collaborate with clinical and administrative team members to support continuity of care
  <\/p><\/li>

• Support communication with patients in both English and Spanish<\/b>

  • Active <\/span>Michigan Family Nurse Practitioner (FNP) license<\/b>

  • Master’s Degree in Nursing or related field
    <\/li>
  • Bilingual in English and Spanish<\/b>
    <\/li>
  • Strong clinical judgment, patient advocacy mindset, and commitment to relationship -based care
    <\/li><\/ul>

    Preferred<\/b><\/span><\/span>
    <\/span><\/span><\/span><\/p>

    • 2–5 years of experience in <\/span>Primary Care<\/b>
      <\/li>
    • Experience in outpatient, community -based, or value -based care models
      <\/li>
    • Comfort working in a fast -paced but patient -focused clinical environment
