Fellow, Nurse Practitioner

 We are seeking a dedicated Nurse Practitioner Fellow to deliver comprehensive primary care services within a patient-centered medical home model. This role integrates clinical expertise with mentorship, education, research, and administrative responsibilities, aligning with Pennsylvania Nurse Practitioner guidelines and ethical standards.

The ideal candidate will excel in effective verbal and written communication, demonstrate advanced interpersonal skills, and uphold strict confidentiality and security standards for patient health information. The position also offers robust opportunities for professional growth through hands-on clinical experience in diverse specialty areas, including women’s health, pediatrics, behavioral health, HIV/Hepatitis C care, and geriatrics, as well as participation in didactic learning sessions and quality improvement initiatives.

Key Responsibilities:

• Provide expert-level primary care and specialty services, including diagnosing and treating acute, chronic, and long-term health conditions.
• Participate in weekly educational sessions and Project ECHO initiatives focused on chronic disease management and addiction treatment.
• Engage in quality improvement programs, leveraging data-driven models and collaborative leadership.
• Perform comprehensive medical histories, physical exams, and care coordination for high-risk patients.
• Prescribe medications, including scheduled substances, per Pennsylvania licensure guidelines.
• Collaborate with interdisciplinary teams to ensure the highest standards of patient care and service delivery.

• Ability to manage emergency care and ensure timely patient documentation.

? Buy in and experience working in the EOS® model (strongly preferred)

? Mission-oriented; represents the enterprise in a professional manner while demonstrating organizational pride

? Must have completed an accredited, Board-approved masters or post masters nurse practitioner program or other Board approved program that awarded an advanced degree or course of study considered by the Board to be equivalent to that required for certification in PA at the time the course was completed.

? Licensed by the State of Pennsylvania as both a Registered Nurse (RN) and a Certified Registered Nurse Practitioner (CRNP)

? Valid DEA Registration Certificate

? Demonstrated expertise in relevant medical practices, protocol, trends and best practices in clinical areas assigned. Demonstrated knowledge and success in overall clinical operations.

? Ability to manage multiple responsibilities and emergency situations successfully

? Proficiency in knowledge of office management, use of computer, software, and office machines

? Familiarity with electronic health records, preferably Medent

? Current BLS, ACLS and PALS or NRP certified

? Valid PA driver’s license
