Location:
live within 30 miles of Jackson, MS
Base salary: $280,000–$300,000

Bonus structure: target is up to 20%

  • 1.25%-1.875& quarterly tied to team performance
  • 5-7.5% annual tied to team performance
  • Up to 5% company annual bonus
    

Job Overview:
We are seeking a Lead Palliative Care Physician to join a growing interdisciplinary care team delivering in-home care to patients with complex chronic conditions. This role focuses on providing compassionate, patient-centered palliative care for individuals living with advanced illnesses such as CKD, ESRD, COPD, and CHF.

The physician will serve as an in-market clinical specialist working closely with the Market Physician Executive (MPE) and multidisciplinary care team to support symptom management, advance care planning, and improved quality of life for patients and their families. This position emphasizes population health management, shared decision-making, and coordinated care delivery while driving clinical outcomes for a defined patient panel.

Key Responsibilities:

  • Collaborate with the Market Physician Executive (MPE) and care team to identify patients who qualify for palliative care services
  • Build and manage a patient panel of individuals with complex chronic conditions
  • Provide compassionate palliative care primarily through in-home patient visits
  • Evaluate and treat symptoms impacting quality of life, including pain, pruritus, depression, anxiety, and insomnia
  • Conduct goals-of-care discussions with patients and families and document advance directives using AD/POLST forms
  • Refer patients to appropriate specialists when needed to support optimal clinical care
  • Support patients through an interdisciplinary team including chaplains and other care providers
  • Assist with transitions to hospice care when appropriate
  • Develop and present clinical metrics and outcomes to leadership as requested
  • Drive clinical and financial program outcomes aligned with established Key Performance Indicators (KPIs)
  • Build relationships with local healthcare providers including PCPs, nephrologists, and specialists
  • Participate in regular meetings with clinical leadership to review performance goals and KPIs
  • Educate and support care team members on palliative care principles, hospice care, advance directives, symptom management, and communication strategies

Required Qualifications:

  • Doctorate degree in Medicine (MD or DO)
  • Board certification in Hospice and Palliative Medicine
  • Active and unrestricted state medical license
  • Willingness to obtain additional state licenses as needed
  • Experience applying evidence-based clinical criteria in complex patient populations
  • Strong skills in goals-of-care discussions and advance care planning
  • Experience caring for high-need populations including Medicare Advantage and managed Medicaid patients
  • Knowledge of integrated care models and social determinants of health
  • Expertise in managing medication plans for complex patients
  • Reliable transportation and ability to travel locally for in-home patient visits
