LPN Or RN- Home Care Urgent Need

Client is a 5yo with multiple complications including a trach and a G-tube.

Schedule is Mondays and Tuesdays: 6am to 2pm or 7am to 3pm

Duties

• Perform comprehensive patient assessments including vital signs, physical examinations, and detailed medical histories to inform diagnosis and treatment plans.
• Document all patient interactions thoroughly using electronic health record (EHR) to ensure compliance with HIPAA
• Provide critical care support in emergency situations including airway management, trauma response, acute pain management.
• Accompany client to school while maintaining constant monitoring
• Perform tracheostomy care and management
• Tube feeding and G-tube management
• Medication administration

Requirements

• Valid RN or LPN license in good standing with the appropriate state licensing board.
• Familiarity with EMR/EHR for accurate documentation.
• Ability to perform physical examinations and diagnostic evaluations with precision
• Strong critical thinking skills combined with excellent communication abilities for patient education and team collaboration.
