POSITION SUMMARY:

Provides pharmaceutical outpatient care to the patient, as well as, ensure the accurate and timely preparation, storage, billing, labeling, and distribution of medications in accordance with the outpatient ambulatory department policies and procedures, and state and federal law under the direction of the Pharmacy Director.  This position reports to the Director of Pharmacy Services.  This position provides ambulatory pharmacist services, develops partnerships with patients, and provides medication management services.

QUALIFICATIONS:

Required:

Three (3) years ambulatory pharmacy experience; Knowledge of pharmacokinetics, drug utilization review, drug distribution systems; Excellent verbal and written communication skills; and maintain an Immunization Certification.

Preferred:

Experience in a similar ambulatory pharmacy practice setting is desirable.  Board of Pharmacy Licensed Preceptor.  Hospital pharmacy background and experience. Board certification in Ambulatory Care or Pharmacotherapy. Certified Diabetes Educator.

Lic/Reg/Cert:  Possess a current Oregon Pharmacist license or eligible for reciprocity.

TYPICAL PHYSICAL/MENTAL DEMANDS:

Light to medium physical demand.  Must be able to stand/sit for periods of time at computer terminal.  Communication with nursing staff and physicians orally, via phone and in writing when needed.  Daily supervision of the ambulatory services staff.  Patient counseling on prescriptions.  Computer skills and understanding of general pharmacy systems.  Develop friendly and positive relationships with pharmacy staff and other caregivers.

ESSENTIAL JOB FUNCTIONS:

  • Implement strategies to promote and improve health literacy and medication adherence.
  • Complete comprehensive patient medication history and review.
  • Design, recommend, monitor, and evaluate patient-specific pharmacotherapy. Maintenance of accurate computerized medication profiles.
  • Provide direct patient care based on Medication Therapy Management protocols and Collaborative Drug Therapy Management agreements.
  • Provide concise, applicable, and timely responses to requests for drug information from healthcare providers and patients.
  • Provide patient counseling, on request, and medication-use education to patients and caregivers.
  • Document drug therapy interventions and medication management services in the medical record and report outcome measures as defined by provider protocols.
  • Ensure continuity of pharmaceutical care to and from the acute and ambulatory patient-care settings.
  • Participate in the established process for assessing, managing, and reporting medication errors.
  • Willingness and knowledge to train new employees, students, and pharmacy residents.
  • Comply in all activities with accreditation, legal, regulatory, and safety requirements.
  • Manage time effectively to fulfill practice responsibilities.
  • Manages and communicates the need to purchase, receive, store, and process ambulatory medications.
  • Maintain a professional image.
  • Attends Pharmacy and Ambulatory services staff meetings.
  • Maximize work efficiency through the use of computers and other technologies.
