Position Overview

One of our customers on the Oomple platform is looking for a Primary Care Nurse Practitioner (PCP NP) to provide direct and indirect patient care under the supervision of the Medical Director/Primary Care Provider (PCP). This role involves managing complex clinical cases, performing health assessments, and coordinating patient care to ensure the best possible outcomes.

Qualifications

  • Master’s degree in Clinical Nursing from an accredited Nurse Practitioner (NP) program.
  • 3-5 years of nursing experience (Nurse Practitioner experience preferred).
  • Ability to practice within state guidelines and meet CMS-approved standardized competencies.
  • Experience working with frail or elderly populations (preferred, training provided if necessary).
  • Reliable transportation, valid driver’s license, and state-required liability auto insurance.
  • Medically cleared for communicable diseases and up-to-date immunizations before direct patient contact.
  • Ability to pass a comprehensive background check, including:
  • Federal and state Medicare/Medicaid exclusion lists
  • Criminal history screening
  • Education and license verification
  • Reference check and drug screening

Technical Skills

  • Proficiency in Electronic Medical Records (EMR) systems.
  • Competency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
  • Strong computer literacy and ability to correspond effectively in writing.

Responsibilities

Key Responsibilities

  • Conduct health histories, physical exams, and patient education.
  • Order, interpret, and follow up on lab tests and diagnostic results.
  • Perform selected procedures and document care in the electronic medical record (EMR).
  • Initiate emergency measures as needed.
  • Manage medication orders and diagnostic studies/treatments within attending physician-approved protocols.
  • Assess, initiate, and coordinate patient care daily under the supervision of the attending physician.
  • Refer patients to specialists and other physicians with staff PCP approval.
  • Act as a liaison with healthcare professionals, providing education, communication, and support to staff, patients, and families.
  • Participate in the provider call schedule and attend staff meetings.
  • Assist the interdisciplinary team (IDT) in coordinating 24-hour patient care.
  • Document patient status changes in the medical record and communicate these updates to the care team.
  • Provide care in various settings, including homes, assisted living facilities, nursing homes, and hospitals.
  • Precept medical students and residents during clinical rotations.
  • Educate patients on Advanced Directives, Do Not Resuscitate (DNR) options, and complete necessary documentation in collaboration with a physician.
  • Other duties as assigned.
