Registered Nurse

 

Are you a Registered Nurse passionate about patient advocacy, patient education and working with high-risk infants and children?  Our Care Management Team is seeking an RN to collaborate with medical professionals and families to support positive outcomes for our clients. This is an opportunity for you to utilize your non-clinical expertise as a Registered Nurse.

 

In this role, you will conduct assessment-based care with high-risk infants, (age 0-12 months), as well as other youth and adolescents in the program. You will provide medical screenings, point-of-contact, and coordination of care with managed care organizations, obtain medical consent reviews and secure approvals.  This role requires community-based service provision in the home of our child welfare service recipients for a portion of your work time and you will have on-call duties, via telephone, as needed.

Responsibilities

Minimum Qualifications

  • Must be a Registered Nurse with valid licensure.
  • Must be available for on-call phone consultations as needed.
  • Must be able to maintain compliance with attending agency required training programs.
  • Proficient use of desktop and laptop computers, smartphones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
  • Valid driver’s license.
  • Reliable personal transportation.
  • Good driving record.
  • Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability.
  • Must have a reliable, working telephone.
