About the Role

Circle Health is looking for compassionate and detail-oriented Virtual Care Coordinators to support physician partners and senior-care practices across India..

As part of the Chronic Care Management (CCM) team, you will help patients with long-term conditions such as diabetes, hypertension, and heart disease remain connected, supported, and compliant with their care plans.

This role involves monthly patient outreach, care coordination, documentation, and communication between patients, nurses, and healthcare providers.

Key Responsibilities:
- Conduct structured monthly check-ins with CCM patients via phone
- Review medications, vitals, and health goals while updating EMR records accurately
- Track preventive screenings, labs, and follow-up activities
- Coordinate between patients, nurses, and providers for appointments and care gaps
- Maintain accurate documentation as per CMS CCM guidelines
- Log activities and time spent for compliance and reporting purposes
- Identify patient concerns or clinical red flags and escalate appropriately

Requirements
- 2–5 years of experience in healthcare coordination, telehealth, medical assisting, or related healthcare experience mandatory
- Experience in CCM is an added advantage
- Strong verbal and written communication skills in English
- Familiarity with EMR systems
- Associate degree or certification as a Medical Assistant, RN, NCLEX, or any other Medical certification.
