Care Coordinator

Education/Experience

Minimum Required:

• Associate's Degree in a Human Service‐related field
• May accept 5+ years of related care coordination experience in lieu of an
  Associate's Degree

Preferred:

• Bachelor's Degree in Science or Arts in Human Services related field

License/Certification

Work Environment

• Combination of Community and Office setting.
• Office setting includes a desk with a computer and a telephone. Community setting includes driving to and entering into organizations and homes in the community to meet partners or program members. Access to personal protective equipment available, though largely not required for position duties.
