What You’ll Do 

 

Position Summary


The Clinical Pharmacist will play a critical role in executing Suvida’s Healthcare model to strengthen the provider-patient relationship and engagement by enhancing the quality and affordability of care. This individual will focus on supporting providers and patients through medication management support, pharmacist-led clinical programs, and practice transformation. You will be responsible for optimizing medication therapy to improve clinical outcomes such as blood pressure and A1c control, reduce preventable ED and hospital utilization, and monitor adherence to medication regimens and care plans. The clinical pharmacist meets the patient where their needs are (virtually, in-office, and at-home) to identify, resolve, and prevent medication-related problems. As time allows, the pharmacist will also participate in various committees and initiatives (e.g. program development for chronic diseases (i.e. COPD, DM, CHF), HEDIS strategies to close gaps, clinic operations, etc.). 

 

Responsibilities

  • Manage pharmacist consults for chronic care management of certain disease states, post-discharge medication management, and other curbside consults
  • Leverage various technology stack (Carium, Elation, etc.) to manage longitudinal care for patients enrolled in their program
  • Complete comprehensive medication reviews while navigating multiple sources of truth (EMR, pharmacy, patient, etc.) and ultimately creating and following up on a care plan with the patient and other care teams as well as the provider team
  • Efficiently assess prescribed medication therapy for unnecessary medications, need for additional medications, ineffective medications, inappropriate doses, adverse drug reactions, and adherence concerns
  • Collaborate with providers, interdisciplinary care teams, patients and/or caregivers to identify and develop patient-specific disease state therapy goals
  • Provide patient-centered education on chronic disease states including medications used for prevention and treatment  
  • Provide patient care services through provider-delegated collaborative practice agreements (e.g. refill approvals, therapeutic interchange, disease state management) 
  • Assist in the development and presentation of educational materials for patients and providers 
  • Participate in quality improvement programs and initiatives driven by clinical needs
  • Maintain comprehensive knowledge base on all heath plan pharmacy benefits
  • Serve as a drug information resource to Suvida Healthcare’s care team and patients.
  • Participate in patient discussions as a member of the multidisciplinary team
  • Support and maintain excellence of Suvida’s clinical programs – identifying staff and practice training opportunities with the intent to optimize the patient experience and quality of care
  • Ensure your team, peers, and patients always a positive experience by providing a high level of service promptly, professionally, and empathetically
  • Demonstrated ability to work independently, solve problems, and make sound decisions in a timely fashion
  • Possess both verbal and written communication skills, interpersonal relationship skills and problem-solving skills

 What You’ll Bring

 

Knowledge, Skills, and Abilities

  • Pharm D. degree or equivalent education in pharmacy 
  • Active, unrestricted Pharmacy license in Texas, or the ability to obtain one

Education, Experience, Licensure, or Certification Requirements

  • 3+ years of clinical pharmacist experience or equivalent residency experience 
  • Advanced in Microsoft office platforms including word, excel, and power point 
  • Bilingual/Bicultural (English and Spanish)
