Position Title: Clinical Pharmacist
Department: Pharmacy
Purpose: The Clinical Pharmacist will be responsible for the provision of pharmaceutical care in all areas of the hospital. The primary responsibility will be to provide clinical pharmacy services to select patient care disciplines that require specialized pharmacy knowledge and practice expertise to ensure the appropriate and cost effective use of medications. The Clinical Pharmacist will be responsible for reviewing patients medication therapy for appropriateness, performing medication use evaluation, facilitating discharge prescription orders, providing educational in-services about drug therapy to physicians, nurses and patients, providing identification and resolution of adverse drug events and adverse drug reactions, providing pharmcokinetic monitoring and renal dosing, facilitating with TPN ordering and support, and providing clinical leadership to other members of the pharmacy team. There may be distributive responsibilities as part of the position.

Supervisory Responsibility: Direction: The job requires some accountability for scheduling, assigning or coordinating work. Employees check the quality of work and provide guidance, instruction, training and direction to others. Although the job does not require formal or official supervisory responsibility, the incumbent serves as group leader or acts in an informal "assistant supervisory" or "lead" role. In addition, the employee may be expected to provide information or suggestions on human resources matters.
Materials Responsibility: Limited: Work requires limited responsibility for material resources. Examples of resources could include supplies, equipment, inventories, small budgets, and other similar material assets. The employee has a limited amount of control over these resources. The cost of errors is also limited in terms of damage, waste or financial loss. Problems associated with material resources are not complex. The volume of resources may vary, but the level of difficulty in dealing with these resources is uncomplicated.

POSITION SPECIFICATION
Education: Bachelor's Degree
Field Of Study: Pharmacy
Training Preferred: Satisfactory completion of ASHP (Accredited Residency in Hospital Pharmacy Practice); clinical pharmacy practice setting experience; or specialized residency and/or board certification; PharmD preferred
Licensure/Registration: Registered Pharmacist in Iowa. Pharmacist applicants obtaining their initial licensure must obtain their Iowa Pharmacist Registration within 6 months of hire. Those applicants registered in another state must obtain their Iowa Pharmacist Registration within 6 months of hire.
Experience: More than 1 year experience required.
Interpersonal Skills: Interaction is with a variety of people inside or outside the organization. Communications are of moderate difficulty and sensitivity. Contact with others may involve detailed & lengthy dialogues & exchanges of information. Requires a moderate amount of interpersonal skills. Interactions involve dealing with moderately complicated problem situations or stressful encounters.
Physical Demands: High Intensity: Work requires heavy physical exertion and effort. Forces exerted are equivalent to lifting between 25 to 50 pounds. The need for physical stamina and endurance is great. The degree of physical strain produced on the job is high. It results in fatigue, and requires periods of rest. Job requirements for manual dexterity and physical manipulation are high. Work could involve the coordination of body movements involving various body postures. The position exceeds these high intensity demands occasionally, 10-35% of the time.
Working Conditions: There is limited exposure to moderately adverse and undesirable environmental conditions. There are some health and safety risks. Position may require safety equipment and precautions. The amount of time the employee may experience these moderate conditions is limited to 35% or less of their work day.
