This job assumes responsibility for the medication management of assigned patients; monitors and evaluates medication therapy for appropriateness based on the patient's health status, institutional criteria /guidelines, and cost efficient measures including medication reconciliation, medication monitoring and patient education; remains integrally involved in the education of Pharmacy and Medical Residents and provides education to health care providers as needed; and provides pharmacy expert content for order sets, benefit design, formularies, and other pharmacy related documents as needed.The Pharmacy Department is made up of separate inpatient and outpatient pharmacies that provide administrative, clinical, and distributive services to both hospital and clinic patients. Pharmaceutical Specialties include: Transplant, Cardiology, Geriatrics, Anticoagulation, Critical Care, Infectious Diseases, Pediatrics, Internal Medicine, and Oncology. Ochsner Ambulatory Pharmacy Services includes Anticoagulation Clinic, Digital Medicine, Population Management, and Pharmacy Patient Assistance.
Ochsner provides internship and externship programs for Pharmacy Students as well as (ASHP) – accredited, PGY-1 and PGY-2, Residency Programs in: Pharmacy Practice, Internal Medicine, Solid Organ Transplant, and Health System Administration. Our clinical team provides training and development for Ochsner’s clinical staff, students, and residents
Education
• Graduate of an accredited School/College of Pharmacy
• Completion of a PGY-1 Pharmacy Residency required; Completion of a PGY-2 Pediatric Specialty Residency or Fellowship preferred
• Licensed to practice pharmacy in the State of Louisiana or eligible for licensure
Certifications
Required - Current Pharmacy license in the state of practice
