Dietician Role $1,850 per weekAbout the RoleWe are seeking a compassionate and dedicated
Registered Dietitian to join our
Program of All-Inclusive Care for the Elderly (PACE) team. The Dietitian will play a key role in supporting participants' nutritional needs by developing individualized care plans, conducting assessments, and collaborating with the interdisciplinary team to promote health, independence, and quality of life.
Responsibilities- Conduct comprehensive nutrition assessments for PACE participants.
- Develop, implement, and monitor individualized nutrition care plans.
- Provide nutrition education and counseling to participants, caregivers, and staff.
- Collaborate with physicians, nurses, social workers, therapists, and other care team members to ensure integrated care.
- Monitor participant progress and adjust care plans as needed.
- Ensure compliance with federal, state, and PACE program requirements.
- Participate in care team meetings and contribute to interdisciplinary planning.
Qualifications- Bachelor's or Master's degree in Nutrition, Dietetics, or related field.
- Current Registered Dietitian (RD/RDN) credential.
- State licensure (if required).
- Minimum 1-2 years of experience working with older adults, preferably in a long-term care, rehabilitation, or PACE setting.
- Strong interpersonal, communication, and teamwork skills.
- Ability to educate and motivate participants and caregivers regarding nutrition and lifestyle changes.
