Mathiesen Memorial Health Clinic is a non-profit Tribal Federally Qualified Health Center owned by the Chicken Ranch Rancheria band of the Me-Wuk Indians. We are nestled in the picturesque Sierra Nevada foothills and just 60 minutes from Yosemite National Park. MMHC is a rapidly growing medical team specializing in primary care, dermatology, gastroenterology and hepatology, behavioral health, addiction treatment, acupuncture, and dentistry. Position Summary
The Family Nurse Practitioner (FNP) will provide comprehensive primary care services to patients across the lifespan, focusing on prevention, wellness, diagnosis, and management of acute and chronic conditions. The FNP works collaboratively with physicians, behavioral health providers, and other members of the care team to improve health outcomes and promote community wellness.
Essential Duties
- Provide patient-centered primary care to individuals of all ages in an outpatient setting.
- Conduct health assessments, order and interpret diagnostic tests, and develop evidence-based treatment plans.
- Prescribe medications under the California scope of practice and clinic protocols.
- Provide patient education and counseling on health maintenance, disease prevention, and chronic disease management.
- Collaborate with tribal programs, community health workers, and public health services to ensure coordinated care.
- Document all encounters and care plans in the electronic health record (EHR) on time.
- Participate in quality improvement initiatives, case reviews, and staff meetings.
- Maintain professional competency and stay current with best practices in primary care and culturally competent care delivery.
- Performs other duties as assigned by the Chief Medical Officer.
Education and Skills
- Current and unrestricted California RN and NP license.
- National board certification as a Family Nurse Practitioner (AANP or ANCC).
- Current DEA registration and furnishing number (or ability to obtain before hire).
- Current BLS certification (ACLS preferred).
- Minimum 1 year of primary care experience preferred.
- Experience working with Native American communities and/or underserved populations preferred.
- Demonstrated ability to provide culturally competent care and build respectful relationships with diverse populations.

- Repetitive motions for computer equipment use.
- Occasional lifting to 25 pounds.
- The ability to deal well with time constraints and emotional stress is necessary.

Working Conditions
Outpatient clinic setting. Monday through Friday, no weekends. While performing the duties of this job, the employee is regularly exposed to odors and airborne particles, as well as the potential risk of airborne pathogens. Exposure to body fluids and viruses is also a potential hazard of this job. The noise level in the work environment is usually moderate.
