Family Nurse Practitioner

This position may be based out of our Ridgeway or Bassett Virginia location. Connect Health + Wellness is committed to providing primary health, dental and integrated behavioral health services at our two Federally Qualified Health Center sites and our Dental Clinic by promoting health, reducing health risk factors, and increasing access to medical dental and behavioral health services, primarily for the uninsured and underserved in our service area. Additionally, Connect Health + Wellness offers chronic disease self-management, medication assistance, and care coordination services to patients and residents within our service area.

Our Family Nurse Practitioners provide comprehensive, evidence-based, primary and preventive medical care in a team-based environment to a diverse patient population with a wide range of health conditions and concerns. Perform physical examinations and preventive health measures within prescribed guidelines and instructions of Physician.

This position will be on day shift and may possibly work three 12-hour shifts per week or four 9-hour shifts per week based on our current needs.
