Benefits
- Base salary: $120,000 - $130,000 , with 10%-15% performance-based bonuses

- Support from an experienced clinical team (MA's with 10-15 years of experience)
- In-house credentialing team
- Average 10 to 15 patients per day
- Focus on chronic disease management
- Develop strategies to reduce ER visits and hospitalizations
- Collaborate with Case Managers and Specialists
- Educate patients on preventive care and treatment adherence
- Experience in value-based care - Required
- Licensed PA in the state of Florida - Required
- Fluent in English and Spanish - Required
- Experience with eClinicalWorks (EMR) - Preferred
- Passion for caring for a senior population
