We are seeking a skilled andcompassionate Nurse Practitioner or Physician Assistant to join our growing team in Brooklyn, NY. Position Details:
- Schedule: Monday-Friday
- No nights, no weekends
- Value-Based Care modelfocused on Geriatric patients
- Small patient panel: Dailycensus of 12-16 patients
- Emphasis on quality careover volume
- No lengthy credentialingprocess
Key Responsibilities:
- Provide primary andpreventive care services to elderly patients in an outpatient setting
- Conduct physical exams,diagnose and treat acute and chronic conditions
- Order and interpretdiagnostic tests, prescribe medications, and manage treatment plans
- Educate patients on healthmaintenance and disease prevention
- Collaborate with physiciansand other healthcare providers to coordinate care for PACE Participants
- Maintain accurate,up-to-date patient records in the electronic medical record system
Compensation & Benefits:
- Compensation: $155,000 - $170,000
- Annual bonus (up to 20% ofsalary)

Requirements:
- Current Nurse Practitioner or Physician Assistant licensure in New York State
- National board certification(FNP, ANP, AGNP or NCCPA)
- At least 1-2 years ofclinical experience preferred (new graduates with strong clinical rotations maybe considered)
- Excellent communication,organizational, and clinical skills
- Ability to workindependently while collaborating effectively with a care team
