Coverage Needed:
- As soon as credentialed - ongoing
- Monday-Friday: 8am-5pm in office
- Overnight call responsibilities begin at 5 PM and end at 8 AM for provider on call
- Weekend call: 1 weekend per month (virtually)
- Weekday call: 2-3 times per week
Clinical Information:
- Support current providers with patient care due the the influx in patient referrals
- Patient volume: 18-22 per day in office
- Support Staff: Medical Assistants, Triage Nurses, on-site cancer center pharmacist, Nurse Navigation
Required:
- CV must be in Month / Year format
- Must have license in hand at time of submission, or proof that the license application has been submitted
- Any gaps greater than 30 days will require an explanation on the CV
- DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
- Active IN License
- Active DEA of one easily transferrable
- EPIC experience - Preferred
- Board Certified
