Job Description:
- Provide comprehensive primary care services to patients aged 18 and older.
- Manage chronic conditions, acute care, and womens health.
- Develop and implement individualized treatment plans.
- Monitor patient progress and adjust care plans as needed.
- Work alongside a board certified internal medicine physician and 2 nurse practitioners to ensure high-quality care.
- Educate patients on health promotion, disease prevention, and management of chronic illnesses.
