Home Health LPN - PACE Program
Coverage Area: Urvada, CO Littleton, Golden, and east to I-25
Schedule: Sunday-Thursday, 7:00 AM - 3:30 PM
Caseload: 5-15 patients per day

We are seeking an experienced Licensed Practical Nurse (LPN) to join our PACE Home Health Program. This role provides direct patient care in the home setting, focusing on:

• Wound care
• Insulin administration
• Medication management and patient education

Requirements:

• Active LPN license in Colorado
• Previous home health experience strongly preferred
• Reliable transportation for coverage across the service area
• Strong clinical and communication skills
