Requirements:
- Active LPN license in Michigan
- Current BLS certification
- Minimum 1 year of LTC or SNF experience preferred
- Strong clinical and communication skills
- Ability to work independently and as part of a team
Responsibilities:
- Administer medications and treatments according to physician orders
- Perform routine health assessments and monitor resident conditions
- Document care and updates accurately in the EMR system
- Assist with ADLs and collaborate with the care team
- Ensure compliance with HIPAA, infection control, and safety protocols
- Respond to urgent medical needs and provide basic life support
- Perform other duties as directed by the supervisor
