Job Title: Nurse Practitioner

Department: Clinics

Status: Full Time, Days

Schedule: 40 hours per week


Summary/Objective

Documents thorough face to face assessments which are approved by the Medical Director to continue care. Candidate must hold a valid Illinois NP/PA license and current BLS and ACLS certifications.


Essential Functions

  • Maintains licensure and collaborative agreement with Medical Director
  • Collaborates with the Medical Director regarding patient appropriateness if any questions arise regarding the patient.
  • Completes documentation of evaluation and assessment of patients.


Experience/Education Requirements

  • Experience in assessing and evaluating patients. 


Licenses/Certifications

  • State of IL and/or IN Nurse Practitioner license

Iroquois Memorial Hospital is a 25 bed hospital located in Watseka, Illinois. The hospital facility is comprised of clinical and diagnostic services which are dedicated to providing high quality health care services to the community. These services include but are not limited to an Emergency Room, full service OR suites, an inpatient care unit, therapy services, advanced medical imaging, Home Health, and Hospice Care. The hospital operates rural health clinics in Watseka, IL, Gilman, IL, Milford, IL, Saint Anne, IL and Kentland, IN. The hospital also operates a skilled nursing facility, Iroquois Resident Home, which is attached to the hospital property.
