Nurse Practitioner - Primary Care

We are seeking a dedicated Nurse Practitioner (NP) to join a collaborative outpatient primary care team.

Key Responsibilities:

• Provide primary care for routine and acute conditions.
• Conduct annual wellness visits and health risk assessments.
• Coordinate patient care with specialists and healthcare teams.
• Educate patients on diagnoses, treatments, and care plans.
• Support population health initiatives and patient outreach.

Required Qualifications:

• MSN, DNP, NP Certificate qualification.
• National certification in an eligible NP specialty or Physician Assistant certification.
• Active, non-probationary state NP license (for NPs).
• Active DEA license.
• Authorization to work in the United States.
• Fluency in Spanish is required.

Preferred: Experience in primary care, internal medicine, geriatrics, or family medicine, with knowledge of ICD-10 coding, HCC documentation, and health risk assessments.
