The Hampden County Sheriff's Office is seeking a compassionate, dedicated, and highly qualified Advanced Practice Provider (Physician Assistant or Nurse Practitioner) to join our Health Services team. Key Responsibilities:

  • Comprehensive Medical Care: Provide general medical care, including primary and secondary preventative health services, emergency care, and chronic disease management for the inmate population.
  • Diagnostic and Therapeutic Procedures: Perform diagnostic and therapeutic procedures traditionally handled by physicians, ensuring a high standard of patient care.
  • Addiction Medicine Collaboration: Work closely with addiction specialists and community health providers to develop and implement individualized treatment plans, including acute withdrawal management and addiction stabilization.
  • Continuity of Care: Assist in transitioning patients to residential treatment facilities and ensure seamless continuity of care upon reentry into the community.
  • Multidisciplinary Collaboration: Participate in team meetings with physicians, nurses, mental health clinicians, and addiction specialists to optimize patient care and treatment strategies.
  • Medical Record Maintenance: Maintain meticulous medical records to ensure accurate documentation and compliance with healthcare regulations and standards.
  • OTP Implementation: Β Possess familiarity with common practices of an Opioid Treatment Program (OTP) in accordance with state guidelines. Prescribe pharmaceuticals in accordance with established MAT guidelines, including experience with all three FDA approved medications for MAT for OUD, including buprenorphine, methadone, and naltrexone.Β Assesses a patient’s physical and psychosocial health status through obtaining the health and illness history and performing a physical exam that incorporates MAT guidelines.

Work Schedule:

Current shift: Monday-Friday with one Saturday per month and rotating shared on-call coverage; must be able to work any shift as directed
