I. SCOPE OF WORK:

Services are required at the Yakama Indian Health Center (Yakama Service Unit) for pharmacy support

services (locum pharmacist) for eligible patients of the Yakama Indian Health Center.

II. OBJECTIVE:

To provide pharmacy support services five days per week. The purpose of the work is to improve the overall health care status of the eligible American Indian, Alaska Native patient population served. Most contacts with patients are through phone or in person. Additional support involves processing prescriptions, checking prescriptions, and counseling. The primary purpose is to provide prescriptions and services to the patients.

III. PERIOD OF PERFORMANCE, LOCATION AND PAY:

Services will be performed at the following location:

Yakama Indian Health Service

401 Buster Road

Toppenish, WA 98948

The contractor will be getting paid a total of 80 an hour.

IV. STATEMENT OF WORK:

A. The Contractor shall provide pharmacy support services during clinic operating hours as scheduled not to exceed 40 hours per week. Regular hours of the Pharmacy are from 8:00 a.m. to 6:30 p.m. The clinic is not open on federally recognized holidays or certain day(s). Inclement weather conditions or facility issues (electrical, water, etc.) may also hinder the normal operating hours and notice of closure will be provided as soon as is practicable by the Yakama Service Unit administrative staff.

B. The contractor will perform pharmaceutical services to focus on therapeutic drug monitoring. Analyze medication regimens to ensure the most effective, least toxic, and most economical treatment plan. Evaluate medication regimens for appropriateness, drug interactions, therapeutic duplications, non-compliance and adverse drug events. Serve as a therapeutic consultant to the medical staff. Accurate filling and dispensing of prescription and over-the-counter drugs on orders of physician, optometrist, physician assistant, family nurse practitioner, and dentist. Counseling patients on the use, storage, cautions and side effects of all medications dispensed as well as documentation of cognitive services provided.

C. Contractor should be proficient in pharmacy database such as/similar to Electronic Health Records (EHR) and RPMS.

D. The Contractor shall be responsible for applicable state and local sales or other taxes as required, except as provided by the Government, to provide professional outpatient family practice services to eligible beneficiaries. Eligible beneficiaries are defined by the policy and procedures of the Yakama Indian Health Service.

E. The Contractor shall provide proper and adequate pharmacy services in accordance with the

Standards of Care established by recognized health care organizations including those of the

Indian Health Service.

V. INDIAN HEALTH SERVICE RESPONSIBILITIES:

A. The Yakama Indian Health Service shall provide all medical and non-medical equipment and supplies used in the Indian Health Service pharmacy for patient care.

B. The Yakama Indian Health Service shall provide an on-going method of monitoring and evaluation of the quality and appropriateness of patient care, and of identifying deficiencies in the quality of services performed before the level of performance becomes unacceptable.

C. Each pharmacy contractor shall be provided with an orientation to the Indian Health Service and the Indian Community being served. The initial orientation and continuing orientation shall be provided by the Yakama Indian Health Service personnel.

D. The Yakama Indian Health Service will be responsible for informing the Contractor of any facility closure due to conditions described in Section IV.A. above as soon as is practicable. Such notification methods shall be mutually agreed upon between the Contractor and the Yakama Indian Health Service.

VI. QUALIFICATIONS:

A. The contractor shall maintain the following qualifications as a minimum:

  • Be a graduate of an accredited pharmacy program/school.
  • State licensed in pharmacy within the United States.
