Pharmacist - Per diem staff pharmacist position
Qualifications include:
*Graduate of a National Association Boards of Pharmacy (NABP) accredited School of Pharmacy.
*Licensure in the State of Wisconsin as a Registered Pharmacist.
*CPR Certified.
*Demonstration of twelve (12) months of recent and relevant experience.
*Ability to interact with all levels of staff in a positive, effective manner.
*Demonstration of ongoing, continuing education with regard to the practice of pharmacy.
*Ability to work within the EPIC medical record system and have had prior experience with an electronic EMR system.
Role summary:
Under the direction of the Director of Pharmacy, the Associate Pharmacist shall provide quality pharmacy services to patients and families. Included in these services, but not limited to, are the appropriate and accurate distribution of medication, the dissemination of patient and staff medication information, the dissemination of medication compatibility, stability, and sterility information, and the monitoring and managing of medication interactions (food-medication information, disease state-medication, age-medication, etc.) Overall, our internal pharmacy mission is to achieve maximal health care outcomes for our patients by envisioning, planning, implementing, and embracing the concept of pharmaceutical care.
