Physical Therapist Role $2,500 per week

Join a mission-driven team providing comprehensive care for older adults in a PACE (Program of All-Inclusive Care for the Elderly) setting. We're hiring a Physical Therapist who is passionate about functional mobility, fall prevention, and helping seniors stay safe and independent.
What You'll Do:

• Deliver physical therapy services in the PACE center, participant homes, and other community locations
• Conduct evaluations, develop treatment plans, and coordinate care with an interdisciplinary team
• Focus on prevention, independence, and quality of life - not productivity quotas
• Participate in team meetings and provide input on long-term care strategies

What You'll Need:

• CO Physical Therapist license in good standing
• Minimum 1 year of experience, preferably with older adults or in a community-based setting
• Excellent interpersonal and collaboration skills
• Comfort with home visits and real-world rehab solutions
