Full-time opportunity for a Board-Certified or Board-Eligible Internist or Family Medicine physician with geriatrics experience Provide overall medical direction and supervision for a Program of All-Inclusive Care for the Elderly (PACE) Administrative reporting to the Executive Director; clinical oversight of Primary Care Providers and Advanced Practice Providers Deliver comprehensive care to seniors age 55+ who qualify for nursing home level care but live in the community Conduct initial and periodic medical assessments, develop individualized care plans, and provide direct patient care as needed Participate in intake and assessment of new applicants; coordinate closely with caregivers and the interdisciplinary team Discuss diagnoses, prognosis, and care options with participants and families; help participants plan future care based on their wishes Collaborate on nursing and interdisciplinary treatment plans and care conferences Oversee and maintain medical policies, procedures, and clinical protocols; lead quality assurance and performance improvement initiatives Educate patients, families, and staff regarding chronic disease management and health maintenance Supervise the care provided by the interdisciplinary team; participate in provider on-call rotation as required MD or DO with unrestricted (or eligible) state medical license; experience in geriatric care or PACE program preferred Competitive base salary in the $240,000"$270,000 range plus performance incentives and comprehensive benefits Visa sponsorship not available Supportive, mission-driven team environment with opportunities for professional growth
