• Types of Cases: Hematology/Oncology managing existing panel and new patients, infusion orders required
• Credentialing: 60-day timeframe; emergency privileges available

Minimum Requirements

• Board Certified/Board Eligible: Must be BC in Hematology/Oncology
• Certifications: BLS
• Licensure: WA license or IMLC accepted
