The Population Health Pharmacist for Stellus Rx will help our communities thrive as a key member of the Adhere Operations Team. ACCOUNTABILITIES
• The Population Health Pharmacist plays a pivotal role in developing, implementing, and overseeing strategies aimed at improving healthcare outcomes for a defined population. This role involves leveraging pharmaceutical expertise, data analysis, and managerial skills to optimize medication therapies, enhance patient adherence, and collaborate with multidisciplinary teams to improve overall health outcomes.


ROLE AND RESPONSIBILITIES:
• Develop, implement, and evaluate population health initiatives centered around medication management, adherence, and
optimizing therapeutic outcomes.
• Utilize health data analytics to identify trends, gaps in care, and opportunities for intervention to improve medication-related outcomes for the target population.
• Collaborate with healthcare providers to ensure appropriate medication selection, dosing, and monitoring, considering population-specific needs and evidence-based guidelines.
• Design and implement programs to educate patients about their medications, ensuring understanding, adherence, and management of potential side effects.
• Work closely with healthcare teams, including physicians, nurses, social workers, and other healthcare professionals, to coordinate care plans and interventions for improved patient outcomes.
• Participate in quality improvement projects aimed at enhancing medication-related outcomes, reducing adverse events, and promoting evidence-based practices.
• Stay updated with pharmaceutical regulations, ensuring compliance with relevant laws, policies, and guidelines.
• Provide leadership within the pharmacy team, mentorship to junior staff, and contribute to departmental goals and initiatives.


MINIMUM QUALIFICATIONS AND REQUIREMENTS:
• Doctor of Pharmacy (PharmD) degree from an accredited institution.
• Active pharmacist licensure in the state or region of practice.
• Advanced training or certification in population health management or a related field (preferred).

• Strong analytical and data interpretation skills.
• Experience in pharmacy practice, preferably in a population health or managed care setting.
• Excellent communication, collaboration, and leadership abilities.
• Knowledge of healthcare regulations, policies, and quality improvement me
thodologies.
