- Schedule: Monday–Friday | Approximately 25 visits/week

Opportunity Overview
We are seeking a compassionate and organized Registered Nurse (RN) to provide high-quality home health care and support patients in achieving better health outcomes. This field-based role offers the opportunity to build meaningful patient relationships while maintaining a balanced Monday–Friday schedule.
Key Responsibilities
- Manage and coordinate care for assigned home health patients.
- Educate patients and families on disease management and care plans.
- Perform skilled nursing assessments, treatments, and follow-ups.
- Monitor patient progress and provide patient-centered care.
- Complete accurate clinical documentation and assessments.
- Collaborate with healthcare teams to support continuity of care.
Required Qualifications
- Graduate of an accredited nursing program.
- Active Colorado RN License in good standing.
- Minimum 1 year of RN experience required.
- Home Health experience and OASIS knowledge are a plus.
- Strong communication and organizational skills.
- Valid driver’s license and reliable transportation for community-based visits.
