Registered Nurse – Community Support Program

Hiring: Registered Nurse (RN) | New York, NY 10026

We are looking for a compassionate and dedicated Registered Nurse (RN) to support individuals transitioning from hospitals and institutional settings into independent community living.

Location: New York, NY 10026
Employment Type: Full-Time
Schedule: Monday-Friday | 9:00 AM - 5:00 PM
Salary: $92,000 annually

Requirements:
 Active New York State RN License
 Nursing experience preferred
 Behavioral health, community health, case management, or home health experience is a plus

Key Responsibilities:
• Conduct comprehensive health assessments
• Support discharge and transition planning
• Develop and manage individualized care plans
• Coordinate primary and specialty healthcare services
• Perform medication reconciliation and patient education
• Collaborate with interdisciplinary healthcare teams
• Connect individuals with community resources and support services
