Schedule: Monday-Friday | 9:00 AM-5:00 PM
Salary: $90,000-$95,000 annually

Job Description

We are seeking a compassionate and dedicated Registered Nurse (RN) to join an Assertive Community Treatment (ACT) Team serving individuals with severe mental illness in the community. The RN will provide psychiatric and medical nursing services while collaborating with a multidisciplinary team to support recovery and improve health outcomes.

Responsibilities

• Conduct psychiatric and medical assessments
• Administer and monitor medications
• Perform physical health evaluations and monitor vital signs
• Participate in treatment planning and care coordination
• Provide individual, family, and group interventions
• Conduct community visits and outreach services
• Assist with crisis intervention
• Coordinate laboratory work and health screenings
• Maintain accurate EMR documentation
• Collaborate with hospitals, physicians, pharmacies, and community providers
• Participate in daily multidisciplinary team meetings
• Provide rotating 24/7 on-call coverage
• Deliver community-based mental health services

Qualifications

• Bachelor's Degree in Nursing (BSN)
• Active New York State Registered Nurse (RN) License
• Minimum one (1) year of experience working with individuals experiencing severe mental illness
• ACT Team experience preferred
• Strong computer and documentation skills
• Valid New York State Driver's License
• Reliable personal vehicle required
