Job Summary: • Provide comprehensive nursing care to patients recovering from illness, injury, or surgery in a rehabilitation setting • Collaborate with interdisciplinary team members to help patients regain independence and functional abilities • Deliver patient-centered care with a focus on restoring mobility, managing pain, and achieving long-term recovery goals
Responsibilities:
Assess, plan, implement, and evaluate individualized care for rehabilitation patients Monitor patient progress and adjust care plans accordingly Administer medications and treatments as prescribed Assist patients with mobility, daily living activities, and rehabilitation exercises Educate patients and families on recovery expectations and home care strategies Collaborate with physicians, therapists, case managers, and other healthcare professionals Maintain accurate and timely documentation in the patient's medical record Promote a safe and supportive environment that encourages healing and patient empowerment Respond to changes in patient condition and take appropriate actions Adhere to rehabilitation nursing best practices and safety protocols
