In this role, Care Managers - RNs will utilize their expertise to create comprehensive care plans that address the unique needs of each patient. By closely monitoring patients' conditions and providing proactive interventions, they can identify potential issues before they escalate, thus reducing the likelihood of hospital admissions. By collaborating with healthcare professionals and coordinating necessary services, they strive to optimize patients' well-being and enhance their quality of life.

Requirements:

This position requires a nurse to have graduated from an accredited School of Nursing and be currently licensed as a Registered Nurse in the State of IA (or another NLC state). Preference is given to candidates with 2+ years of experience in care management and/or working with patients with CKD/ESRD. The nurse should be able to take call remotely on some nights and weekends and must have experience in Home Health and/or Hospice.

Responsibilities:

  • Develop and update personalized care plans with physicians.

  • Conduct regular in-home visits to execute care management plans.

  • Act as the main point of contact for members, answering their health-related questions.

  • Prepare care recommendations and escalations, write reports, and communicate them in weekly internal case rounds.

  • Prioritize visits, recommendations, and focus based on the needs of managed care clients.

  • Engage with plan members and caregivers using effective communication skills and patience.

  • Provide support during transitions like post-op and hospital discharge visits.

  • Document updates and progress of plan members in the care management platform.

  • Monitor biometric data and take necessary actions based on approved protocols.

  • Coordinate with pharmacists and prescribers to manage medication inventory.

  • Conduct health assessments and surveys as required.

  • Provide education on chronic kidney disease (CKD), end-stage renal disease (ESRD), dialysis, and related conditions.

  • Encourage medication and treatment adherence through frequent contact with members.

  • Collaborate with social workers to meet the social and behavioral needs of plan members.

  • Facilitate discussions and educate members about proactive care decisions, such as transplantation, home modalities, and AV fistula placement.

  • Ensure smooth transitions of care by coordinating with dialysis providers.
