We are seeking a dedicated Registered Nurse (RN) to manage a caseload of individuals enrolled in the Home and Community-based Services (HCS) Program. This is a contract role that focuses on delivering high-quality, person-centered nursing care to individuals with intellectual and developmental disabilities (IDD) living in home and community settings in North Texas.
The RN will serve as the primary clinical resource for assigned individuals, ensuring compliance with state regulations and promoting optimal health outcomes.
Responsibilities
- Manage a designated caseload of individuals receiving HCS services
- Conduct initial and ongoing comprehensive nursing assessments
- Develop, implement, and revise individualized nursing service plans
- Oversee medication administration and monitor for effectiveness and side effects
- Provide direct nursing care as needed in residential settings
- Delegate nursing tasks to unlicensed assistive personnel in accordance with Texas Board of Nursing rules
- Train and supervise direct care staff on health-related supports and protocols
- Monitor chronic conditions and respond to acute health changes
- Coordinate medical care with physicians, therapists, case managers, and guardians
- Participate in team meetings and contribute to individualized plans
- Ensure compliance with Texas Health and Human Services Commission (HHSC) regulations and HCS program requirements
- Maintain accurate and timely documentation, including nursing notes, incident reports, and health records
- Conduct routine health audits and quality assurance checks
