  • License: Insert Texas license status or requirements if known
Clinical Setting
  • Practice Type: Primary Care Physician's Office
  • Practice Size: 3 Physicians
  • Patient Volume: 20-30 patients per day, per provider
  • Age Range: 12 years and older
Clinical Scope
  • Services Include:
    • Preventative care and routine check-ups
    • Minor injury treatment
    • Immunizations
    • Well-woman exams
    • EKGs
    • Chronic illness management (but not chronic medication management)
    • Referrals to specialists
  • Clinical Focus: MIPS Performance & Quality Measures
