Requirements:
- Current and valid LPN license in Ohio
- Minimum of 1 year of experience in a long-term care or similar healthcare setting preferred.
- Basic Life Support (BLS) certification required.
- Strong communication, organizational, and interpersonal skills.
- Ability to work effectively in a team-oriented environment and handle multiple priorities.
- Compassionate and patient-centered approach to care.
Responsibilities:
- Deliver direct nursing care, including vital sign monitoring and medication administration.
- Assist residents with activities of daily living (ADLs).
- Collaborate with the healthcare team to develop and follow care plans.
- Accurately document patient care and health status changes.
- Ensure compliance with infection control and facility policies.
- Maintain a safe, clean, and supportive environment.
- Provide emotional support and communicate with residents and families.
