Grade/Salary Range: C41($28.78 - $35.67/hour) / C42 ($30.15 - $37.38/hour) - Non-exempt salary commensurate with experience Position Summary: Under the direction of the TB Clinic and Communicable Disease Nurse Supervisor, the Public Health Nurse is responsible for providing community outreach nursing care to clients in Montgomery County based on established written nursing procedures and Lead case guidelines. Responsibilities include but not limited to: Providing health and community resource education to clients/families on health promotion and/or concerns Completing home visits and phone consultations as needed for reported lead based cases; Submitting required forms and reports based on assessments and findings from home visits and/or phone consultations Performing Lead case education for identified children with elevated lead blood levels in Montgomery County Performing health screenings at various sites throughout Montgomery County Providing vaccines to home bound eligible clients and at various sites Conferring with staff, doctors, and providers regarding client health issues Assisting with duties related to the immunization mobile unit Maintaining OSHA, CLAI and HIPAA standards for all clients/families Minimum Qualifications: PH Nurse I/C41: Bachelor's degree in Nursing, OR Associate's degree in nursing with 2 years of relevant experience Current valid licensure from State of Ohio to practice as a Registered Nurse Must obtain 24 CEU's every two (2) years Understands and acknowledges that Public Health - Dayton & Montgomery County uses evidence-based and evidence-informed public health practices based on scientific research including health promotion theory and practice, environmental health measures, and communicable disease control measures including vaccination, isolation, and quarantine practices, and medicine for treatment and prevention of disease, among others. Understands and acknowledges the use of tobacco or nicotine products in any form including replacement therapy products and prescription medications used to treat smoking addiction for anyone hired after January 1, 2014, is prohibited Required to play an active role during a public health emergency, crisis, outbreak, incident, or event in which the Health Commissioner deems additional personnel resources are warranted.
